5. Filing a DMCA Counter-Notice

A valid counter-notice must include:

  • Your full legal name and signature

  • Complete contact information

  • Identification of the removed material and its prior URL location

  • A statement under penalty of perjury asserting that the removal resulted from mistake or misidentification

  • Consent to the jurisdiction of the appropriate U.S. Federal District Court and acceptance of service of process from the original complainant
